APPOINTMENTS & REGISTRATION

If you would like to make an appointment to see one of our practitioners for an orthotic or prosthetic consultation, please fill out the Contact Us form. If you are a vendor, and would like to make an appointment with one of our practitioners, please fill out the Contact Us form. 

Once the appointment request has been received via email; we will gladly call your home or office to set up your appointment or scheduled meeting.

For patients, once your appointment is set; you may print out the adult or pediatric registration welcome packet and forms which will inform you of your privacy rights and OPE facility practices.

Please fill out these forms and bring them with you to your scheduled appointment. Remember to bring your insurance cards, prescription referral, and to arrive 15 minutes early. All deductibles, coinsurance and co-payments are due when services rendered.
