Appointments & Registration

If you would like to schedule an appointment with one of our practitioners for an orthotic or prosthetic consultation, or if you are a vendor and would like to schedule an appointment with one of our practitioners, please complete the Contact Us form. 

Once the appointment request has been received via email, we will gladly call your home or office to set up your appointment or scheduled meeting.

For patients, once your appointment is scheduled, you may complete the patient information form, which will inform you of your privacy rights and OPE facility practices. 

Please complete the forms and bring them with you to your scheduled appointment.

Please remember to bring your insurance cards, prescription referral, and arrive 15 minutes early. All deductibles, coinsurance, and co-payments are due at the time services are rendered.
